Reducing Inpatient Hospital Readmissions with Analytics

Healthcare expenditures are on the rise, in part, due to aging populations and patients with chronic diseases, which make up 40% of the total U.S. population and account for 75% of healthcare spending. Join Tableau & Divurgent for a discussion on how leveraging predictive data analytics and dashboards can prevent unnecessary inpatient hospital admissions, deploy preventive care management programs, and better to be able to engage with your members outside of traditional care settings. Topics discussed in this webinar include:

  • Reducing unnecessary and costly hospital admissions
  • Identifying at-risk members for proactive outreach by Care Managers
  • Increase member satisfaction and retention
  • Join us to learn more about data-driven solutions for payers and how it helps improve the patient experience at your organization.
